Documentation in the event of total permanent invalidity due to illness

A situation in which the insured person is affected by permanent Disability for all types of work due to illness.

All of the following documentation must be submitted to the office:

  • Case history and/or Medical report in which the following are specified:
    • Background, exact date of diagnosis and evolution, as well as possible antecedents with their dates of diagnosis.
    • Treatment followed.
    • That there is evidence of absolute and permanent disability for all types of work.
  • Report from the Medical Disability Assessment Unit (UMVI) of the Social Security, Ministry of Social Affairs or other competent body.
  • In the event that there is no Determination of Disability by a Competent Body:
    • The medical history should also include the evolution and current situation and an indication of the possibility of recovery and the definitive consequences.

Documentation in the event of death of the insured person due to illness

All of the following documentation must be submitted to the office:

  • Official death certificate for the insured person.
  • Medical certificate and/or medical history from the doctor who has attended the insured person, indicating the cause, exact date of diagnosis and the evolution of the illness(s).

Documentation in the event of the Death of the Insured person due to Accident (external, sudden and violent cause beyond the control of the Insured person)

All of the following documentation must be submitted to the office:

  • Official death certificate for the insured person.
  • Police and/or judicial proceedings carried out at the accident.
  • Autopsy and Toxicological analysis if performed.

ADDITIONAL DOCUMENTATION accrediting the status of the beneficiary to be provided if a claim is accepted in the event of the insured person's death

  • Photocopy of the identity card of the beneficiaries.
  • Photocopy of the Family Book if the beneficiaries are the spouse, children or parents. Where this is not the case, proof of identity and, where applicable, of the rights of the beneficiaries.
  • Certificate of the Last Will and Testament.
  • Photocopy of the Will if it was made after the designation of beneficiaries in the policy.
  • Presentation or Payment of the Tax on Inheritance and Gifts (this last requirement will be provided once the Company has confirmed the payment of the benefit and requested it).
